Vellum; 7 membranes, 4.68 × 0.27 m. On the recto, 589 shields arranged in 88 rows (Cooke's Ordinary); on the dorse of the last membrane, 1 + 35 shields arranged in 5 rows of 7 (Balliol Roll)

Owned by the Franciscan friars of Carmarthen (16th-cent. mention in Welsh on the dorse); Robert Cooke, Clarenceux King of Arms (d. 1593); Sir Thomas Phillipps (d. 1872): his MS. 26463; Sir Anthony Wagner, Garter King of Arms (d. 1995)
